Assessment portal to measure quality of care

Our client, the nonprofit Altarum Institute, in collaboration with the Health Care Incentives Improvement Institute, Inc. (HCI3), is dedicated to improving the quality and affordability of healthcare in the U.S. The organizations implement evidence-based incentive programs for healthcare facilities and clinicians and offer a comprehensive package of solutions aimed at improving patient care while reducing costs.

One of the projects Altarum and HCI3 sought to implement was a solution designed to assess the quality of care. The primary goal was to assess and promote high-quality care for patients with chronic conditions, including asthma, heart failure, depression, and diabetes.

EffectiveSoft had previously worked with HCI3, successfully completing a project for the organization. Impressed by our proficiency and expertise, the client turned to us to implement this new solution.

After thoroughly reviewing the functional requirements and design wireframes provided by the client, our engineers designed and implemented the Bridges to Excellence (BTE) Scoring Assessment Portal, a web-based analytical application for medical assistance appraisal. The BTE portal is built around Care Recognition programs, each featuring a set of clinical measures that reflect that standard of care for patients with specific chronic conditions (asthma, heart failure, depression, diabetes, hypertension, etc.). For each Care Recognition program, our team implemented scoring algorithms and input formats.

Using the portal, healthcare practices and clinicians can apply for Care Recognition certification to prove that they provide safe, timely, efficient, equitable, and patient-centered care. Applicants who meet the defined care benchmarks are certified for superior care performance under various incentives.

Features

Registration of medical facilities and workers. The portal provides both individual and group assessments—an entire medical practice can register or physicians can register separately.

Selection of Care Recognition programs. The platform has different Care Recognition programs based on various conditions, allowing registrants to apply for one program or several at once.

Assessment of the provided medical data. Based on the selected program, users fill out a CSV file and upload it to the platform. The system assesses the submitted data against various measures and determines the recognition status level in accordance with the earned points for each measure.

Reports on the assessment results. Users are provided with detailed assessment reports identifying areas of excellent and poor patient care delivery.

Certification of medical facilities and clinicians. An official certificate is issued for each recognized healthcare practitioner and medical practice.

The engineers at EffectiveSoft successfully delivered a comprehensive web platform to measure the quality of care provided by healthcare specialists. The BTE Scoring Assessment Portal provides rapid and independent assessment and rewards excellence in the area of patient care of chronic conditions, promoting reliability and trust-based relationships between healthcare providers and patients.
